A patient's ankle swells on Saturday. They call their doctor's office, it is closed. They drive to the emergency room, wait four hours and pay $2,000 for an x-ray. In 1979, urgent care filled the gap, not emergency, not primary care. The in-between, urgent care centers treated minor injuries without hospital overhead. Sprained ankles, deep cuts, ear infections, strep throat, conditions needing fast treatment, but not intensive care. Urgent care deployed freestanding clinics in neighborhoods stuffed by nurse practitioners at reasonable rates. Insurance loved it, ERs handled true emergencies, everyone else went to urgent care. The unit's economics became the moat.
